Editable Coaching Accreditation Forms

Definition/use of forms:

Assessment Task Descriptions (non-editable): to be provided to participants undertaking a coaching accreditation course.

Assessment Marking Sheets: to be used by assessors to mark coaching accreditation candidates for respective courses.

RPL Application Form: Used by individuals who are seeking recognition of prior learning for coaching accreditation.

For Orienteering Instructor and Learn and Play Orienteering Coach the form should be submitted to your State/Territory Coaching Coordinators
For Competition and Performance Orienteering Coach the form should be submitted to the OA Manager of Coaching Development (coaching@orienteering.asn.au)

RPL Assessment Form: used by coaching coordinators to assess an RPL application. For Orienteering Instructor and Learn and Play Orienteering Coach this will be completed by State/Territory Coaching Coordinators.

Authorisation Form: used by coaching coordinators to authorise coaching accreditation following either successful completion of a coaching accreditation program or application for RPL. For Orienteering Instructor and Learn and Play Orienteering Coach this will be completed by State/Territory Coaching Coordinators.
